GRADUATION REGISTRATION REQUIREMENTS

1. Complete the Graduation Biodata Form.

2. Attach a photocopy of the most recent diploma.
3. Attach a photocopy of the ID card (KTP).
4. Submit one (1) 3x4 photograph to the Academic and Student Affairs Subdivision (Mr. Moch Subiyanto), subject to the following requirements:
   1. Recent photograph (not the S1 photograph), wearing the university blazer (men must wear a black tie; women wearing a hijab must wear a dark/black hijab).
   2. Face the camera directly, with teeth not visible.
   3. Photographs taken using a mobile phone camera are not permitted; studio photographs are recommended.
   4. No hats, caps, or other head accessories are permitted.
   5. No sunglasses are permitted.
5. Complete the Declaration of Commitment to Complete the Alumni Tracer Study.

6. Complete the Faculty of Psychology graduation registration form in MMC.
7. Complete the SKPI form in MMC (for prospective S2 graduates only).
